Active Bucks is a weekly programme of fun and inspiring activities taking place across Buckinghamshire; on days, evenings and weekends. It will continue throughout the year and new activities will be added regularly.
The Active Bucks project aspires to help all Bucks residents lead happier, healthier lives.
We have a walking football session in Stoke Poges on a Thursday and would appreciate help promoting this to the public at Threeways surgery and having GP’s promote this as a suitable form of exercise to refer people onto.

- Active Bucks is a countywide campaign to get more people more active more often
- 1 in 4 people in Bucks are inactive, undertaking less than ½ hour of moderate intensity physical activity a week. Many more are not active at optimal levels for their health.
- Based on this we have now commissioned 142 new activity programmes which each run for 6 months
- These programmes will deliver 3408 Active Bucks activity sessions
- Activities start in May 2016 and run till September 2017
- The Active Bucks activities will cost a maximum of £2 per session, with a number of activities being free of charge.
- A bespoke summary document is attached for the Active Bucks activities close to your practice.
- For up to date information about all Active Bucks activity sessions, including a description of the activity, venue, time etc. go to buckscc.gov.uk/activebucks.
- 19 Community champions (volunteers) have been recruited to support the sustainability of the project
- In addition we are launching a countywide Active Bucks promotional campaign in June at activebucks.co.uk to help patients to find an activity of their choice that they will enjoy, within 20 minutes of their home or work, on a day and time that suits them, from a database of thousands of activities that charge a variety of prices, as well as the Active Bucks activities.
- The campaign offers a free first activity session from over 1500 different activities across Bucks, through registering for a voucher.
Active Bucks commissioned Activities range from Rounders, Walk/Jog/Run groups, Family Cycling, Buggy Fit, Orienteering/Scavenger Hunt, Scooter Skills, Tai Chi/Pilates, Olympic Multisports, Parkrun/Junior Parkrun, Dads and Toddlers, Parent Fit, Strength and Balance, Parkour, Flag Football. Baseball & Softball, Walking Football, Social Ballroom, Multisport Activities, Yoga/Pilates, Bootcamp, Running Group, Nordic Walking, Simply Walks, Walking Netball, Dance. Bushcraft, Conservation, Photography lessons, Environmental art, Mystery trail, Live Action Role Play (LARP), Quidditch, Woodland/NERF games.

The Live Well Stay Well single point of access celebrates it’s first birthday this month. The service has triaged over 3000 patients to a selection the interventions on offer and outcomes show an impressive increase in patient motivation to embrace a healthier lifestyle following their support from the services.
Health Care Professionals can refer to Live Well Stay Well with the single referral form on their practice system or patients can self-refer via the phone number or website. The services on offer are:
- Tier 2 weight management - 12 week funded programme with Weight Watcher, Slimming or other providers
- Type 2 Diabetes Education – Life and Health programme group education provided by BHT or digital/telephone led service from Oviva Diabetes Support
- Health Coaches - 121 healthy living support, advice, goal setting in a community location that is local and accessible to the patient
- Exercise Referral – low cost gym programme for patients with LTC
- Support to cut down alcohol intake/ dependency
- Support for anxiety and depression through Healthy Minds (Live Well)
- Support with the emotional challenges of managing a long term health condition
Our Practice Nurse can refer you to any of the above on a single referral form. Let Live Well Stay Well discuss with you what you are eligible for and the best way to support you.
LWSW also phones patients 7 days, 6 weeks, 3 months and 9 months after their initial triage to see how they are progressing and if they need any further support.
